Current balances
Most recent three months' bank statements for all account, including IRA, 401-K an retirement funds, mutual funds or investment accounts
Type and curent value of Stocks and Savings Bond information

Income / Employment

Empoyment: Two year history. (Name, address, position, length of time on job)
Income: base earnings, average overtime, commisssions, bonuses, etc., current pay stub(s) from the last 30-day period, last two years W-2 forms.
If self-employed, bring copies of last two years' completed federal income tax returns (personal and corporate).
